Certification Forms

The following forms are needed to apply for certification.  Only these forms will be accepted.  If you have questions regarding the forms, please forward your question CERTBOARD@gmail.com.

1)    Certification Application Form (complete version) – completed by all practitioners.  The full Supervisor Assessment form is included in this packet.  The Portfolio, below, will need to be added to this set of forms. 

2)  Portfolio  – This must be completed by all applicants.  This is provided in WORD doc file format to make it easier to fill out and submit. 

3)   NACP Child and Youth Care Competencies - The work and document developed by the Association for Child and Youth Care Practice and the North American Certification Project that serves as one of the primary sources for C&YCCB testing and certification.

4)   ACYCP Child and Youth Care Ethics - The Code of Ethics for the profession of child and youth care.
